Coordinated Entry 

Coordinated Entry is the gateway to housing and supportive services for individuals and families experiencing homelessness in Lee County. If you are living outdoors, in a vehicle, or in an emergency shelter, we encourage you to complete a Coordinated Entry assessment. This assessment helps identify your housing needs and connect you with available community resources.

To speak with staff and complete an assessment, call 239-533-7996 Monday through Friday, from 8:00 a.m. to 4:30 p.m.

During the assessment, you will be asked a series of questions to better understand your housing situation and needs. The assessment takes approximately 15 minutes to complete.

Please note that completing a Coordinated Entry assessment does not guarantee housing assistance. While participating in Coordinated Entry is an important step, we encourage you to continue seeking other resources and housing options to help resolve your housing crisis.

Housing Outreach and Treatment Teams

In Lee County, HOT Teams are referred to Housing, Outreach, and Treatment teams to encapsulate the scope of services being provided. Teams are a collaboration between Lee County, Centerstone, and local law enforcement, including:

  • Lee County Sherriff's Office, 
  • Cape Coral Police Department, 
  • Fort Myers Police Department, 
  • Sanibel Police Department, 
  • Lee County Port Authority and 
  • Florida Gulf Coast University Police Department.

Lee County's HOT Teams conduct street outreach to persons experiencing homelessness, with the goal of connecting individuals and families to housing and services. These teams conduct assessments, provide referrals for housing and shelter, and connect households with behavioral health and other supportive services.
