When is my second dose Appointment?
- Individuals who receive their first dose will leave the site with a scheduled second appointment noted on their vaccination card.
- The second dose date written on your vaccination card is a
confirmed appointment. You do not need to receive a confirmation call, email or text to appear on the date and time written on your vaccination card.
What to bring to your second dose appointment
If you received your first dose before January 21
- Individuals who received their first dose from FDOH Lee before January 21, 2021 that have not received their second-dose should send an email to
