The Florida Department of Health in Lee County (DOH-Lee) is offering COVID-19 vaccine to high-risk frontline health care workers and those 65 and older. The registration system is operated by:

Know before you call. Making COVID-19 vaccination appointments at state-supported sites. 1) Call: Calling the automated hotline number and providing your contact information & date of birth will place you in a queue to receive a call-back for scheduling. 2) Answer: Every eligible individual will receive a call-back from a representative to schedule a vaccine appointment. 3) Arrive: arrive on time for your vaccination appointment. Remember to bring a form of identification, proof of residency & CDC appointment card if it is your second dose.

Ways to receive a call-back for vaccine appointment scheduling. How eligible individuals will be contacted after pre-registering when vaccine appointments are available. 1) Call: Several attempts will be made to call-back an individual for appointment scheduling. A voicemail will be left if the individual does not answer. 2) Text: Representatives may reach out to eligible individuals via text to assist in appointment scheduling. 3) Email: Representatives may reach out to eligible individuals via email and provide a call-back number to assist in appointment scheduling.

When is my second dose Appointment?

  • Individuals who receive their first dose will leave the site with a scheduled second appointment noted on their vaccination card.
  • The second dose date written on your vaccination card is a confirmed appointment. You do not need to receive a confirmation call, email or text to appear on the date and time written on your vaccination card.

If you received your first dose before January 21

  • Individuals who received their first dose from FDOH Lee before January 21, 2021 that have not received their second-dose should send an email to
